"Prior to drafting any contract for goods or services, the responsible party or the decedent's survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent."
We are pleased to furnish the following information relating to the various services that we offer so that you maybe better inform and thus enable to make those important decisions in a way which is most beneficial to you. The following prices and infomation are made available as required by the Federal Trade Commission.
ln certain instances, the Federal Trade Commission requires us to use detinite and specific wording. Some of these phrases may not always apply to Groman Mortuary, lnc. ln such case your counselor will try to explain any item that you find confusing, However, nothing the counselor or anyone else may say should be taken as modifying or differing from the written words below and on the contract statement describing the funeral goods and services selected, The contract will show a simple form as possible, your selections, prices, and goods and services which are included. You will be given the opportunity to review the contract before slgning it a copy will be given to you. The goods and services shown are those we can provide to our customers.
You may choose only the items you desire. lf legal or other regulations require that you must buy any items that you did not speclfically or ask for, we will explain the reason in writing on the statement we provide for describing the funeral goods and services you selected. However, any funeral arrangements you select will include a charge for our services. (The list does not include certain items that you may ask us to obtain on your behalf. The prices for those items will be shown on the contract or statement describing the funeral goods and services you have selected) A basic fee is charged to every family for our professional expertise in assisting them with their preferences.
It includes a proportionate share of the business expenses required to serve the public. The following group of services and facilities includes the usual items which make up the complete traditional funeral services,

MINIMUM SERVICES OF THE FUNERAL DIRECTORS & STAFF
The fee for our basic services will be added to the total cost of funeral arrangements you selected. This fee lncludes the following: Personnel available 24 hours a day, 365 days a year to respond to initial call. Arrangement Conference Coordinating services plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased. Clerical assistance in the completion of various forms and permits associated with a funeral. Also covers overhead, includlng facility maintenance, equipment and inventory cost, insurance and administration expenses, and government compliance. THESE DISCLOSURES ARE REQUIRED BY AND CONFORM WTH 1984 FEDERAL TRADE COMMISSION RULES RELATED TO FUNERAL INDUSTRY PRACTICES.
The goods and services shown herein are those we can provlde to the families we serve, You may choose only those items you desire. Embalming may be necessary how ever, if you select certain funeral arrangement that embalming is not required such as direct cremation or immediate burial, without ceremony. If embalming is declined and the final disposition is not completed within 8 hours from the time of death, we require refrigeration.

CASH ADVANCE ITEMS
Inorder to better serve your needs, Groman Mortuary, Inc. makes the following items available to you.The charge for each depends on the cost changed to us of that particu lar item.W.hen you request any of these items, the estimate or exact charge will be ·shown on the statement (contract) that your funeral counselor will provide. In addition to the items below, in some cases it is necessary to advance cemetery charges, coroner fee's, charges for
special services or charges from other mortuaries and will be charge to you accordingly.
PRIEST/ PASTOR
Many families wish us to have the Priest/ Pastor at the service. Church / Priest/Pastor specify their fee,s upon request to use their service.We simply include this fee on ou r contract and present the officiating Priest/ Pastor with a check at the time of the service. The family has option to contract and pay their own Priest/ Pastor directly.
OBITUARIES
We are able to assist the family on composing an obituary notice to appear in any publication. Our standard placement includes a two line notice in the Los Angeles Times and other newspapers to be charge in their contract if they requested. Placements in other publications will vary, depending on their actual charges, to be charged to your contract.
PRAYER CARDS
Prayer Card - Black & White = $40.00 per 50 pieces. $80.00 per 100 pieces
MEMORIAL FOLDER - Black & White = $100.00 per 100 pieces
REMOVAL
Removal of remains to mortuary within 25 mile radius $450.00. Over 25 mile radius will be charged at a rate of $1.50 per loaded mile. The 450.00 fee is included in the Professional Service Charge. This does not include distances beyond 25 miles. Oversize Removals from 300 lbs and over: Charges $ 800.00
REFRIGERATION $125.00 (DAILY BASIS)
Remains without embalming can be kept in refrigeration for 125.00/a day or a maximum of $1,250.00/ 100 days.
MISCELLANEOUS EXPENSES
Picking up clothing, getting documents signed, delivery of cremains, delivery of death certificates, etc. is $85.00 within a 25 mile limit. Beyond 25 mile limit will be charged at a rate of $1.25 per mile, one way.
SHIPOUT SERVICES
Securing of Burial Ship out documents at Latin American Consulate is a minimum charge of $200.00. Once the certificate and burial transit permit is being obtained, our staff must go to the corresponding consulate to present those documents and pay the necessary fees to obtain the transit documentation. If the decedent is exiting the U.S. died of a non-contagious disease; our staff must get a letter of approval from the local Health Department with a $20.00 fee of the said document, which has to be attached and presented to the consulate office requiring said documents. If the decedent is to leave to a Latin American country, like Mexico, our staff must translate all the documents, Apostille and submit them together to the consulate, however, only some consulate require translation DF Mexico ship out requires Apostille documents. (Minimum Charge of $200.00) charges vary on different consulate offices and our counselor will give the charges.
OVERNIGHT SERVICES
Family could request for overnight services including the services of a counselor responsibility for the service and a security guard provided is a charge of $1,500.00/ per overnight service.
This amount is additional to the actual service charge in the contract.
AIRLINE CHARGES ARE ESTIMATED
Family may request a Saturday, Sunday or Holiday service, which Groman Mortuary, Inc., who will provide attending personnel work overtime supervising the service. There will be no additional charge if services is on regular days

EMBALMING:
EMBALMING of INFANT/ CHILD from O to 10 years old $ 200.00
Expected In certain cases, embalming is NOT REQUIRED by law. Embalming may be necessary however, if you select certain funeral arrangement that embalming is not required such as direct cremation or immediate burial, without ceremony. If embalming is declined and the final disposition is not completed within 8 hours from the time of death,we require refrigeration.

Groman Mortuaries can provide, at the request of the family, a limousine.
This charge provides for 3 hours or 75 miles (time and mileage is calculated from the home base of the vehicle) or part thereof. Each additional hour or 75 miles (or part thereof) maximum charged at the rate of $700.00

CASH ADVANCE ITEMS
In order to better serve your needs, Groman Mortuary,Inc. makes the followingitems available to you. The charge for eachdepends on the cost to us of that particular item.When you request any of these items,the estimated or exact charge will be shown on the statement (contract) that your funeral counselor will provide. In addition to the items below, in some cases it is necessary to advance cemetery charges, coroner fee's, charges for special services or charges from other mortuaries.
PRIEST / PASTOR
Many families wish us to provide the Priest / Pastor for the services they perform. A Church’s Priest or Pastor specify their fee's upon request to use their service. We simply include this fee on our contract and present the officiating Priest or Pastor with a check at the time of the service. The family has option to contract and pay their own Priest / Pastor directly.
OBITUARIES
We are able to assist the family on composing an obituary notice to appear in any publication.
Our standard placement includes a two line notice in the Los Angeles Times and other newspapers.
This is automatically placed unless the family request otherwise. Placements in other publications will vary, depending on their actual changes and are to be charge to your contract.
PRAYER CARDS/MEMORIAL FOLDER: Black & White = $40.00 per 50 pieces. $80.00 per 100 pieces / Colored = $175.00 per 100 pieces
MEMORIAL FOLDER: Black & White = $100.00 per 100 pieces
REMOVAL
Removal of remains to mortuary within 25 mile radius $250.00.
Over 25 mile radius will be charged at a rate of $1.50 per loaded mile. The 300.00 fee is included in the Professional Service Charge. This does not include distances beyond 25 miles.
REFRIGERATION $ 125.00 Daily Basis. Remains without embalming can be kept in refrigeration for $125.00/a day or a maximum of $1250.00 for 10 days.
MISCELLANEOUS EXPENSES
Picking up clothing, getting documents signed, delivery of remains, delivery of certificates. etc. is $85.00 within a 25 mile limit. Beyond 25 mile limit will be charged at a rate of $1.50 per mile one way.
SHIPOUT SERVICES Securing of Burial/ Ship out documents at Latin American Consulate is a minimum charge of $100.00. One the certificate and burial/ transit permit is being obtained,our staff must go to the corresponding consulate to present those documents and pay the necessary fees to obtain the transit documentation. If the decedent is exiting the U.S.died of a contagious disease like, HIV/ AIDS,our staff must get a letter of approval from the local Health Department with a $10.00 fee or the said document,which has to be attached and presented to the consulate office requiring said documents. If the decent is to a Latin American country, like Mexico, our staff must translate all the documents,and submit them all together to the consulate, however, only some consulate require translation.
OVERNIGHT SERVICES
Family could request for overnight services including the services of a counselor responsibility for the service and a security guard provided is a charge of $750.00/ per overnight service. This amount is additional to the actual service charge in the contract.
SATURDAY / SUNDAY SERVICE $ 375.00
Family may request a Saturday/ Sunday service, which Groman Mortuary, Inc. will provide attending personnel who will work overtime supervising the service. The charge will not apply if all regular days are fully booked and Groman Mortuary, Inc. has no alternative but to render Saturday / Sunday service as a part of a regular service day.
AIRLINE CHARGES ARE ESTIMATED
Family may request a Saturday, Sunday or Holiday service, which Groman Mortuary, Inc., who will provide attending personnel work overtime supervising the service. There will be no additional charge if services is on regular days
